Purpose of the Position: Under the supervision of the CMS manager, assesses clients with chronic medical conditions, identifies their health care needs, and develops and implements plans of care to optimize healthy outcomes. Documents all client encounters and maintains accurate medical records. Provides care coordination services and education to clients and their families with links to community resources and services. Prepares clients and their families for transition to adult care. This position works to promote and protect the health and safety of the residents of Cobb and Douglas Counties.
 
Essential Functions (include, but are not limited to):
  • Receives referrals and makes initial contact with family. Explains purpose and scope of Children’s Medical Services program. Schedules intake appointment and meets with client/family to gather and assess medical and social information, obtaining necessary signatures and authorizations.
  • Consults and coordinates care with internal and external health care professionals to develop and evaluate the individualized and comprehensive plan of care.
  • Contacts family for six-month review and meets with client/family for annual update.
 
RN 2 Minimum Qualifications: Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or one (1) year of experience at the lower level PH Registered Nurse 1 (HCP190) or position equivalent.
          
Preferred Qualifications: Bilingual English/Spanish. Experience with case management, children and adolescents with chronic medical conditions and special healthcare needs.
 
Work Conditions & Physical Demands: General office environment and in-home client visits.

Salary Information:  $61,706.94 Annually / Pay Grade L
