Essential Functions (include, but are not limited to):
- Receives referrals and makes initial contact with family. Explains purpose and scope of Children’s Medical Services program. Schedules intake appointment and meets with client/family to gather and assess medical and social information, obtaining necessary signatures and authorizations.
- Consults and coordinates care with internal and external health care professionals to develop and evaluate the individualized and comprehensive plan of care.
- Contacts family for six-month review and meets with client/family for annual update.
RN 2 Minimum Qualifications: Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or one (1) year of experience at the lower level PH Registered Nurse 1 (HCP190) or position equivalent.
Preferred Qualifications: Bilingual English/Spanish. Experience with case management, children and adolescents with chronic medical conditions and special healthcare needs.

Salary Information: $61,706.94 Annually / Pay Grade L
